Background Information
It has been recognised that many aged care homes are finding it increasingly difficult to keep the services of GPs. There is evidence that:
- The proportion of GPs providing services to aged care homes is shrinking.
- At the same time, the needs of residents, especially in high care, are becoming increasingly complex.
The Aged Care Access Initiative, aims to improve access to Primary Care, both GP and Allied Health services, for residents of approved Aged Care facilities.
The Monash Aged Care Program supports, and is supported by, approved aged care homes within the Division Boundaries
